Summary
Once, there was a demon named Gajasura who performed severe penance. Easwara (Lord Shiva) appeared before him and granted him a boon. Gajasura requested that Easwara merge into his stomach, which Easwara granted.
Parvati, unable to find Easwara, sought help from her brother, Vishnu, who knew of Easwara's habit of granting boons indiscriminately. Vishnu assured her that he would bring Easwara back.
Vishnu transformed Nandi (Siva's bull) into a dancing bull and, as a piper, played music before Gajasura. Gajasura, pleased with the performance, offered the piper (Vishnu) anything he desired. Vishnu asked Gajasura to release Siva from his stomach.
Gajasura, realizing that the piper was Vishnu, fell at Vishnu's feet, released Siva, and requested that his head be worshipped by all after his death. Siva then placed Gajasura's head on his son, Vinayaka.
This is why Easwara wears an elephant's skin, and the head of Gajasura is worshipped as Vinayaka's head.
